These counts and lists are worked out from the library itself each time the page loads. A score rates how much weight a study can bear — its design, journal, evidence and recency — not whether its result is good news. How we score a study.

The 20 highest-scoring studies

  1. Gut microbiota in patients with Alzheimer's disease spectrum: a systematic review and meta-analysis Aging · 2022-01-14 · · Peer-reviewed
  2. The microbiota-gut-brain axis in mild cognitive impairment and Alzheimer's disease: a scoping review of human studies Alzheimer's & dementia : the journal of the Alzheimer's Association · 2026-01-01 · · Peer-reviewed
  3. Tau degradation in Alzheimer's disease: Mechanisms and therapeutic opportunities Alzheimer's & dementia : the journal of the Alzheimer's Association · 2025-03-01 · · Peer-reviewed
  4. A phase 2 double-blind placebo-controlled 24-week treatment clinical study of the p38 alpha kinase inhibitor neflamapimod in mild Alzheimer's disease Alzheimer's research & therapy · 2021-05-27 · · Peer-reviewed

In this 24-week phase 2 trial in mild Alzheimer's disease, the drug neflamapimod did not improve the main or secondary clinical measures compared with placebo. It did significantly lower spinal-fluid levels of total tau and p-tau 181, markers of nerve damage and tau disease. Combined with signals in people with the highest drug levels, the authors argue a longer, higher-dose study is warranted, so any clinical benefit remains unproven.

  5. Frequency of LATE neuropathologic change across the spectrum of Alzheimer's disease neuropathology: combined data from 13 community-based or population-based autopsy cohorts Acta neuropathologica · 2022-06-13 · · Peer-reviewed
  6. Cerebrospinal fluid biomarker panel for synaptic dysfunction in a broad spectrum of neurodegenerative diseases Brain : a journal of neurology · 2024-07-01 · · Peer-reviewed
  7. Probiotic intervention benefits multiple neural behaviors in older adults with mild cognitive impairment Geriatric nursing (New York, N.Y.) · 2023-03-28 · · Peer-reviewed
  8. The intestinal microbiota as an ally in the treatment of Alzheimer's disease Gut microbiome (Cambridge, England) · 2023-06-19 · · Peer-reviewed
  9. Inflammatory bowel disease is associated with higher dementia risk: a nationwide longitudinal study Gut · 2020-06-23 · · Peer-reviewed
  10. Gut microbiota, cognitive frailty and dementia in older individuals: a systematic review Clinical interventions in aging · 2018-08-29 · · Peer-reviewed
  11. Serum IgG antibody levels to periodontal microbiota are associated with incident Alzheimer disease PloS one · 2014-12-18 · · Peer-reviewed
  12. Plasma MTBR-tau243 biomarker identifies tau tangle pathology in Alzheimer’s disease Nature Medicine · 2025-03-31 · · Peer-reviewed

In three cohorts, plasma eMTBR-tau243 was elevated at the mild cognitive impairment stage and increased further in dementia, and it was strongly associated with tau PET binding and cognitive performance, outperforming other plasma tau biomarkers. This is a blood biomarker for detecting and tracking tau tangles, not a treatment, cure or prevention.

  13. Structural progression of Alzheimer's disease over decades: the MRI staging scheme Brain communications · 2022-04-28 · · Peer-reviewed
  14. Sensitivity and specificity of a seed amplification assay for diagnosis of multiple system atrophy: a multicentre cohort study The Lancet. Neurology · 2024-12-01 · · Peer-reviewed
  15. Prevalence and clinical associations of tau in Lewy body dementias: A systematic review and meta-analysis Parkinsonism & related disorders · 2020-09-23 · · Peer-reviewed

Pooling 49 articles, this review found Alzheimer-type tau tangles (Braak stage ≥ III) in 66% of people with dementia with Lewy bodies and 52% with Parkinson's disease dementia at autopsy. In dementia with Lewy bodies, more tau was linked to fewer hallucinations, less parkinsonism, worse cognition before death and lower accuracy of diagnosis during life. The authors conclude tau is common in these dementias and may make dementia with Lewy bodies harder to diagnose correctly during life.

  16. Safety, tolerability and efficacy of the glutaminyl cyclase inhibitor PQ912 in Alzheimer's disease: results of a randomized, double-blind, placebo-controlled phase 2a study Alzheimer's research & therapy · 2018-10-12 · · Peer-reviewed

In a phase 2a trial in people with Alzheimer's, the glutaminyl cyclase inhibitor PQ912 cut the target enzyme's activity, reaching an average target occupancy of > 90%, and was linked to a reduction in EEG theta power and better scores on one memory test. Serious adverse events were slightly more common than on placebo, though not significantly different. The authors see only early signals of efficacy and call for longer studies at lower doses.

  17. Gut Microbiome Alterations in Mild Cognitive Impairment: Findings from the ALBION Greek Cohort Microorganisms · 2025-09-10 · · Peer-reviewed
  18. The Amyloid Hypothesis of Alzheimer's Disease: Progress and Problems on the Road to Therapeutics Science · 2002-07-19 · · Peer-reviewed

This paper describes the amyloid hypothesis, which proposes that accumulation of amyloid beta-peptide in the brain is the primary influence driving Alzheimer's disease pathogenesis. According to the hypothesis, the rest of the disease process, including formation of neurofibrillary tangles containing tau protein, is proposed to result from an imbalance between amyloid beta production and clearance.

  19. β-Secretase Cleavage of Alzheimer's Amyloid Precursor Protein by the Transmembrane Aspartic Protease BACE Science · 1999-10-22 · · Peer-reviewed

This study cloned and characterized BACE, an enzyme with the characteristics of beta-secretase, which cleaves amyloid precursor protein to produce amyloid beta peptide. It suggests that future development of BACE inhibitors may be beneficial for treating Alzheimer's disease, but no treatment was tested in people.

  20. TDP-43 Mutations in Familial and Sporadic Amyotrophic Lateral Sclerosis Science · 2008-02-28 · · Peer-reviewed

This study identified mutations in the TARDBP gene in people with amyotrophic lateral sclerosis (ALS), a motor neuron disorder, and found that mutant TDP-43 caused neural apoptosis in a chick embryo model. It does not test a treatment or cure for Alzheimer's disease.
