These counts and lists are worked out from the library itself each time the page loads. A score rates how much weight a study can bear — its design, journal, evidence and recency — not whether its result is good news. How we score a study. 13. Spatial patterns of neuroimaging biomarker change in individuals from families with autosomal dominant Alzheimer's disease: a longitudinal study The Lancet. Neurology · 2018-02-01 · · Peer-reviewed

In people from families with inherited (autosomal dominant) Alzheimer's disease, brain scans of mutation carriers began to differ from those of non-carrier relatives in a set order: amyloid deposition first (on average 18.9 years before expected onset), then reduced glucose metabolism (14.1 years), then structural decline (4.7 years). These figures are averages across brain regions of when the differences became statistically detectable, not personal timelines, and they varied by several years. The study shows these changes come before the expected start of dementia in this inherited form; it does not show that one change causes the next, or that the same timing applies to late-life Alzheimer's.

  14. Optical coherence tomography in mild cognitive impairment - Systematic review and meta-analysis Clinical neurology and neurosurgery · 2020-06-22 · · Peer-reviewed
  15. Sex Differences in Longitudinal Tau-PET in Preclinical Alzheimer Disease: A Meta-Analysis JAMA neurology · 2025-03-03 · · Peer-reviewed

In this meta-analysis, among people with high amyloid, female sex was associated with faster tau accumulation in certain brain regions, and among APOE ε4 carriers female sex was associated with faster tau accumulation in the inferior temporal region. The authors suggest these sex differences call for sex-specific timing considerations when giving anti-amyloid and anti-tau therapies, but no treatment was tested here.

  16. Frequency and Clinical Outcomes Associated With Tau Positron Emission Tomography Positivity JAMA · 2025-06-16 · · Peer-reviewed

In 6,514 people, a positive tau PET scan was rare before age 50 but was estimated at 19% of cognitively unimpaired people by age 90, and at 79% of people with Alzheimer's dementia at age 75. Among cognitively unimpaired people, those positive on both amyloid and tau PET had an estimated 57% five-year risk of developing MCI or dementia, compared with 17% for amyloid-positive/tau-negative and 6% for people negative on both. The authors note this was a convenience sample and describe tau PET as a promising staging marker. These are associations, not a prediction for any one person.

  17. Non-stationarity in the "resting brain's" modular architecture PloS one · 2012-06-28 · · Peer-reviewed
  18. Combining CSF MTBR-tau243 and plasma pTau217 ratio enhances the prediction of continuous regional tau PET burden in early Alzheimer's disease Alzheimer's & dementia : the journal of the Alzheimer's Association · 2025-11-01 · · Peer-reviewed
  19. Resting-state fMRI in Parkinson's disease patients with cognitive impairment: A meta-analysis Parkinsonism & related disorders · 2018-12-17 · · Peer-reviewed
  20. Brain and blood metabolite signatures of pathology and progression in Alzheimer disease: A targeted metabolomics study PLoS medicine · 2018-01-25 · · Peer-reviewed

In brain tissue and blood samples, certain sphingolipid species were associated with severity of Alzheimer pathology at autopsy and with increased risk of future conversion to Alzheimer disease in cognitively normal individuals. The study notes its relatively small number of brain tissue samples as a limitation.
