Examining multimorbidity contributors to dementia over time
Matthias Klee, Sheila Markwardt, Miriam Elman, Ling Yan Han, Anja K. Leist, Heather G Allore, Ana Romàn Quiñones. Alzheimer s & Dementia, 2025-02-01
Topic: Risk, prevention and care in population studies
Hypertension, acute myocardial infarction, atrial fibrillation, diabetes, heart failure, ischemic heart disease, stroke, and arthritis additively accounted for 71.8% (95% confidence interval [CI]: 62.9%-79.1%) of ADRD incident cases based on LE-AAF. Our findings suggest that multimorbidity plays a pivotal role in ADRD incidence. Targeting constituents of a cardiovascular path to dementia may contribute most to lowering dementia risk. HIGHLIGHTS: Most dementia cases (71.8%) were attributable to eight chronic conditions. Hypertension was the largest contributor to dementia risk. Confidence intervals were smallest for constituents of a cardiovascular path to dementia. Longitudinal extension of the average attributable fractions (LE-AAFs) explicitly consider longitudinal patterns of comorbidities. Acute myocardial infarction did not contribute significantly to dementia incidence.

Key findings
- Hypertension, acute myocardial infarction, atrial fibrillation, diabetes, heart failure, ischemic heart disease, stroke, and arthritis additively accounted for 71.8% (95% confidence interval [CI]: 62.9%-79.1%) of ADRD incident cases based on LE-AAF.
- Our findings suggest that multimorbidity plays a pivotal role in ADRD incidence.
- Targeting constituents of a cardiovascular path to dementia may contribute most to lowering dementia risk.
- HIGHLIGHTS: Most dementia cases (71.8%) were attributable to eight chronic conditions.
- Hypertension was the largest contributor to dementia risk.
Why it matters
This study found that eight chronic conditions, led by hypertension, together accounted for 71.8% of Alzheimer's disease and related dementias cases, with acute myocardial infarction not contributing significantly. It suggests that targeting cardiovascular conditions may contribute most to lowering dementia risk.

How this study scored 52/100
Published in a peer-reviewed journal, and it is a longitudinal study following the same people over time. Scored zero for sample size, studied in people and randomised / blinded because the record does not state it.
