Prognostic value of plasma %p-tau217 in cognitively unimpaired older adults
Jesús Silva-Rodríguez, Linda Zhang, Luca Kleineidam, Francisco J López-González, Sonia Wagner-Reguero, Minerva Martínez-Castillo, Aitana Sogorb-Esteve, Michelle E Guamán-Chulunchana et al.. Alzheimer's & dementia : the journal of the Alzheimer's Association, 2026-07
Topic: Plasma and spinal-fluid biomarkers
Participants with Elevated/High %p-tau217 exhibited accelerated cognitive decline, hippocampal atrophy, and significantly higher risks of progression to MCI (hazard ratio [HR] > 6.0) and dementia (HR > 9.9). The Low group demonstrated a 92.2% 10-year negative predictive value for any progression. Our results suggest that %p-tau217 is a robust prognostic biomarker, accurately distinguishing CU individuals at minimal risk from those at high risk of future AD-related clinical and neurodegenerative decline. Similar performance was achieved by immunoassays.
Key findings
- Participants with Elevated/High %p-tau217 exhibited accelerated cognitive decline, hippocampal atrophy, and significantly higher risks of progression to MCI (hazard ratio [HR] > 6.0) and dementia (HR > 9.9).
- The Low group demonstrated a 92.2% 10-year negative predictive value for any progression.
- Our results suggest that %p-tau217 is a robust prognostic biomarker, accurately distinguishing CU individuals at minimal risk from those at high risk of future AD-related clinical and neurodegenerative decline.
- Similar performance was achieved by immunoassays.

How this study scored 79/100
Published in a leading specialty journal, and it is a longitudinal study following the same people over time. Scored zero for randomised / blinded because the record does not state it. We have no data on journal's citation record, times cited, citations per year, impact for its field and year, cited by studies here, still being cited, senior author's track record, institution's track record, multi-centre collaboration and free to read, so those were left out: it earned 44 of the 56 points we could judge it on, which is 79 out of 100.
