In a medical emergency, call 911. More numbers, and what each line does, on the help page.

A biomarker is something measurable in the body — here, usually a protein in blood or spinal fluid — that tracks with the disease. Two numbers describe how well a test works:

  • Sensitivity — of the people who really have the condition, what share does the test catch? Low sensitivity means missed cases.
  • Specificity — of the people who don't have it, what share does the test correctly clear? Low specificity means false alarms.
  • AUC is a single score combining the two. 1.0 is perfect; 0.5 is a coin flip.

Read the compared against line on each record. A test checked against autopsy is being held to a far harder standard than one checked against another blood test.

How to read the labels: Measured counted, surveyed or written in law. Modelled estimate produced by a statistical model — an estimate, not a count. Basis not stated the source did not say which.

6 of the 95 records behind this page are model estimates, not measurements. They are produced by statistical models — projections, small-area estimates such as CDC PLACES, Global Burden of Disease figures, pooled meta-analyses, or cost models. A model estimate can be the best number available and still be wrong for any particular person, county or year.

Each one carries a Modelled estimate label where it appears.

Fourth FDA-cleared AD blood test; first single-biomarker test cleared for both rule-in and rule-out with the same cutoffs across primary and specialty care; three result categories (positive/intermediate/negative). Press release gives NO performance numbers, only 'high agreement with amyloid PET imaging'. Not stand-alone. See separate row for the prototype-assay clinical-cohort study (Hibar 2026). Strength of the evidence: Moderate (company press release; pivotal data not peer-reviewed at time of capture)

Second FDA-cleared AD blood test; first indicated for primary care; RULE-OUT only. Press release states NPV 97.9% and nothing else (no sensitivity, specificity, PPV, AUC, or reference-standard detail beyond 'Alzheimer's-related amyloid pathology'). NPV in a low-prevalence population is inherently high; PPV was not disclosed. Also CE-marked (IVDR). OpenFDA 510(k) record: K252163 Elecsys Phospho-Tau (181P) Plasma, decision date 2025-10-08 (Roche announced 2025-10-13). FROM 510(k) SUMMARY K252163 (Roche study RD006263, prospective, 8 sites: 6 US, 2 EU; 95.8% US): n=312, mean age 69.1 (55-80), 59% white, 34% Black; 41% SCD, 56% MCI, 1% mild dementia. Reference = amyloid PET visual read (3-reader majority); PET prevalence only 13.1% (41/312). KEY CAVEAT: the headline 97.9% NPV arises mostly from the 13% prevalence; specificity is 51% and PPV 22%, so a positive result is more often wrong than right.

What this means if you are the one waiting for a result

A biomarker result is one piece of evidence. On its own it does not diagnose Alzheimer's, and it cannot tell you when or whether symptoms will start. Ask the clinician who ordered it which test was used, what the result was compared against, and what they would do differently depending on the answer.

This page is a summary of published studies. It is not medical advice and it is not a substitute for the doctor who knows the case.

Evidence page Blood tests for Alzheimer's: what they measure → p-tau217, p-tau181, Aβ42/40, GFAP and NfL, marker by marker, with every figure sourced to a study or an FDA document. Next step What a diagnosis actually involves → Where a blood test fits in the full workup, and what happens after the appointment. Related evidence Brain scans and other tests → PET, MRI and cognitive tests: what each one shows, what it costs, and who covers it.

What could make these numbers misleading

Accuracy figures depend heavily on which reference test was used (amyloid PET, spinal fluid, or autopsy) and on who was in the study. A test validated in a memory clinic can perform differently in primary care. Figures marked 'modelled' are pooled meta-analysis estimates, not a single measured study.

Every figure on this page is read straight out of a file in our data folder; nothing is typed in by hand. You can download the raw data and check any row yourself. This is a summary of published research, not medical advice.
