Limited Longitudinal Change in Self-reported Spatial Navigation Ability in Preclinical Alzheimer Disease
Levine TF, Roe CM, Babulal GM, Fagan AM, Head D.. Alzheimer disease and associated disorders, 2022-01-01
Topic: Amyloid, tau and blood-test diagnosis
Subtle changes in objective spatial navigation ability have been observed in the preclinical stage of Alzheimer disease (AD) cross-sectionally and have been found to predict clinical progression. However, longitudinal change in self-reported spatial navigation ability in preclinical AD has yet to be examined. The current study examined whether AD biomarkers suggestive of preclinical AD at baseline spatial navigation assessment and APOE genotype predicted decline in self-reported spatial navigation ability and whether APOE genotype moderated the association of AD biomarkers with change in self-reported spatial navigation.
Key findings
- However, longitudinal change in self-reported spatial navigation ability in preclinical AD has yet to be examined.
- The current study examined whether AD biomarkers suggestive of preclinical AD at baseline spatial navigation assessment and APOE genotype predicted decline in self-reported spatial navigation ability and whether APOE genotype moderated the association of AD biomarkers with change in self-reported spatial navigation.
- Clinically normal (Clinical Dementia Rating Scale=0) adults aged 56 to 90 completed the Santa Barbara Sense of Direction Scale (SBSOD) annually for an average of 2.73 years.
- Biomarker data was collected within +/-2 years of baseline (ie, cerebrospinal fluid Aβ42, p-tau181, p-tau181/Aβ42 ratio, positron emission tomography imaging with Florbetapir or Pittsburgh Compound-B, and hippocampal volume).
Why it matters
This longitudinal study found limited change over time in people's self-reported spatial navigation ability in the preclinical stage of Alzheimer's disease. It is about a self-reported symptom measure, not about treating, curing or preventing the disease.

How this study scored 43/100
Published in a peer-reviewed journal, and it is a longitudinal study following the same people over time. Scored zero for sample size, effect reported, studied in people and randomised / blinded because the record does not state it. We have no data on senior author's track record, so that was left out: it earned 41 of the 96 points we could judge it on, which is 43 out of 100.
