Longitudinal brain structural changes in preclinical Alzheimer's disease
Jordi Pegueroles, Eduard Vilaplana, Victor Montal, Frederic Sampedro, Daniel Alcolea, Maria Carmona-Iragui, Jordi Clarimon, Rafael Blesa et al.. Alzheimer's & dementia : the journal of the Alzheimer's Association, 2016-09-28
Topic: Plasma and spinal-fluid biomarkers
At follow-up, stage 1 subjects showed reduced atrophy rates in medial frontal areas and precuneus compared to stage 0 subjects, whereas stage 2/3 subjects presented accelerated atrophy in medial temporal structures. Low CSF Aβ1-42 levels were associated with reduced atrophy rates in subjects with normal tau levels and high CSF tau levels with accelerated atrophy only in subjects with low Aβ1-42 levels. Our longitudinal data confirm a biphasic trajectory of changes in brain structure in preclinical AD. These have implications in AD trials, both in patient selection and the use of MRI as a surrogate marker of efficacy.
Key findings
- At follow-up, stage 1 subjects showed reduced atrophy rates in medial frontal areas and precuneus compared to stage 0 subjects, whereas stage 2/3 subjects presented accelerated atrophy in medial temporal structures.
- Low CSF Aβ1-42 levels were associated with reduced atrophy rates in subjects with normal tau levels and high CSF tau levels with accelerated atrophy only in subjects with low Aβ1-42 levels.
- Our longitudinal data confirm a biphasic trajectory of changes in brain structure in preclinical AD.
- These have implications in AD trials, both in patient selection and the use of MRI as a surrogate marker of efficacy.

How this study scored 59/100
Published in a leading specialty journal, and it is a longitudinal study following the same people over time. Scored zero for sample size, effect reported, studied in people and randomised / blinded because the record does not state it. We have no data on journal's citation record, times cited, citations per year, impact for its field and year, cited by studies here, still being cited, senior author's track record, institution's track record, multi-centre collaboration and free to read, so those were left out: it earned 33 of the 56 points we could judge it on, which is 59 out of 100.
