Impact on End‐of‐life Planning of a Care Coordination Program for persons living with dementia

Topic: Risk, prevention and care in population studies
Abstract Background Alzheimer’s disease and related dementias (ADRD) afflict over five million persons in the U.S., and persons living with dementia (PLWD) have been shown to receive poor quality care and have worse health outcomes compared to those without ADRD. Results We enrolled 195 patients in the CARES dementia care program. Of the POLST forms completed, 70.7% indicated that the patient was ‘do not attempt resuscitation’ (DNAR) (65.2% of those who completed the POLST pre‐program, 77.8% for those who completed after enrollment, p = 0.19).
Key findings
- Abstract Background Alzheimer’s disease and related dementias (ADRD) afflict over five million persons in the U.S., and persons living with dementia (PLWD) have been shown to receive poor quality care and have worse health outcomes compared to those without ADRD.
- Results We enrolled 195 patients in the CARES dementia care program.
- Of the POLST forms completed, 70.7% indicated that the patient was ‘do not attempt resuscitation’ (DNAR) (65.2% of those who completed the POLST pre‐program, 77.8% for those who completed after enrollment, p = 0.19).
Why it matters
Among 195 patients enrolled in a dementia care program, 70.7% of completed POLST forms indicated do-not-attempt-resuscitation, with no significant difference between forms completed before versus after enrollment. This concerns end-of-life planning, not treating, curing or preventing Alzheimer's disease.
