FDA Grants Accelerated Approval for ADUHELM (aducanumab) as the First and Only Alzheimer's Disease Treatment to Address a Defining Pathology of the Disease
U.S. Food and Drug Administration (announced via Biogen press release), 2021-06-07
Topic: Amyloid, tau and blood-test diagnosis
On June 7, 2021, the FDA granted accelerated approval to aducanumab (Aduhelm), the first new Alzheimer's drug approved since 2003 and the first ever approved for a mechanism thought to modify disease course. The approval was based only on the drug's ability to reduce amyloid plaques on brain scans, not on proven cognitive benefit, and it required a follow-up trial to confirm real-world benefit. The decision was highly controversial: an FDA advisory committee had voted against approval, and two committee members resigned afterward.
Key findings
- Approved under the accelerated pathway based on amyloid plaque reduction (71% in the EMERGE trial, 59% in ENGAGE, both p<0.0001) as a surrogate marker, not confirmed clinical benefit
- Continued approval was made contingent on a confirmatory post-marketing trial (ENVISION)
- FDA's own outside advisory committee had recommended against approval in November 2020
Why it matters
Aducanumab received accelerated approval based only on its ability to reduce amyloid plaques on brain scans, not on proven cognitive benefit, and continued approval required a confirmatory trial. The decision was highly controversial, with an FDA advisory committee voting against approval and two committee members resigning afterward.

Study details
- Study designregulatory decision
- Effect sizeApproval basis: amyloid-PET reduction of 71% (EMERGE) and 59% (ENGAGE), both p<0.0001, used as a surrogate marker
- Evidence strengthPreliminary
Limitations
This is a regulatory press release, not a primary study; approval rested on a surrogate biomarker rather than proven clinical benefit, and FDA's own advisory committee had recommended against approval. Underlying trial data captured separately (see emerge-engage-aducanumab-jpad-2022).
How this study scored 16/100
Regulatory decision — not a study. It scored 17 on the same rubric, but non-peer-reviewed sources are capped at 16, so it shows as 16. Scored zero for study design, sample size, studied in people, randomised / blinded and indexed in PubMed because the record does not state it. We have no data on times cited, citations per year, impact for its field and year, still being cited, senior author's track record, institution's track record, multi-centre collaboration and free to read, so those were left out: it earned 11 of the 66 points we could judge it on, which is 17 out of 100.
