FDA Approves LEQEMBI (lecanemab-irmb) Under the Accelerated Approval Pathway for the Treatment of Alzheimer's Disease
U.S. Food and Drug Administration (announced via Biogen press release), 2023-01-06
Topic: Amyloid, tau and blood-test diagnosis
The FDA gave lecanemab (Leqembi) accelerated approval based on Phase 2 data showing it reduced amyloid plaque in the brain, ahead of full Phase 3 results. The label carried a warning about ARIA (amyloid-related imaging abnormalities), including brain swelling (ARIA-E, seen in 10% of treated patients vs 1% on placebo) and small brain bleeds (ARIA-H, 6% vs 5%). Full approval was contingent on the Phase 3 Clarity AD trial confirming real clinical benefit.
Key findings
- Accelerated approval based on amyloid-PET reduction from Phase 2 data, not yet on proven cognitive benefit
- ARIA-E in 10% of lecanemab patients vs 1% placebo; ARIA-H in 6% vs 5%; one large intracerebral hemorrhage (>1cm) reported in the treatment group
- Traditional approval was explicitly pending confirmatory results from the Clarity AD Phase 3 trial
Why it matters
Lecanemab received accelerated approval based on Phase 2 data showing it reduced amyloid plaque, ahead of full Phase 3 results, with a warning about ARIA including brain swelling in 10% of treated patients versus 1% on placebo. Full approval was contingent on the Phase 3 Clarity AD trial confirming real clinical benefit.

Limitations
Regulatory press release, not primary research; based on Phase 2 amyloid-reduction data pending Phase 3 confirmation.
How this study scored 15/100
Regulatory decision — not a study. It scored 15 on the same rubric, which is below the 16 cap we apply to non-peer-reviewed sources. Scored zero for study design, sample size, studied in people, randomised / blinded and indexed in PubMed because the record does not state it. We have no data on times cited, citations per year, impact for its field and year, still being cited, senior author's track record, institution's track record, multi-centre collaboration and free to read, so those were left out: it earned 10 of the 66 points we could judge it on, which is 15 out of 100.
| Component | Points | Why |
|---|---|---|
| Journal tier | 2 / 10 | Tier 4 — not a peer-reviewed journal (U.S. Food and Drug Administration (announced via Biogen press release)) |
| Journal's citation record | 0 / 4 | Not a journal article, so the journal's record does not count |
| Study design | 0 / 12 | Design recorded as “regulatory decision”, which our rubric does not rank (not stated in the record — scored 0) |
| Strength of evidence | 2 / 5 | Rated “early” in our summary |
| Peer-review validation | 1 / 10 | Regulatory decision — not a study. Named source is a regulator (FDA / EMA / CMS / SEC). |
