Quantitative estimate of cognitive resilience and its medical and genetic associations
Thanaphong Phongpreecha, Dana Godrich, Eloise Berson, Camilo Espinosa, Yeasul Kim, Brenna Cholerton, Alan L Chang, Samson Mataraso et al.. Alzheimer's research & therapy, 2023-11-06
Topic: Brain imaging, connectivity and assessment
CR varied across five cognitive assessments and was greater in female participants, associated with longer survival, and weakly associated with educational attainment or APOE ε4 allele. In contrast, damage was strongly associated with APOE ε4 allele (P value < 0.0001). Major predictors of CR were cardiovascular health and social interactions, as well as the absence of behavioral symptoms. Our framework explicitly decoupled the effects of CR from neuropathologic damage. Characterizations and genetic association study of these two components suggest that the underlying CR mechanism has minimal overlap with the disease mechanism. Moreover, the identified medical features associated with CR suggest modifiable features to counteract clinical expression of damage and maintain cognitive function in older individuals.
Key findings
- CR varied across five cognitive assessments and was greater in female participants, associated with longer survival, and weakly associated with educational attainment or APOE ε4 allele.
- In contrast, damage was strongly associated with APOE ε4 allele (P value < 0.0001).
- Major predictors of CR were cardiovascular health and social interactions, as well as the absence of behavioral symptoms.
- Our framework explicitly decoupled the effects of CR from neuropathologic damage.
- Characterizations and genetic association study of these two components suggest that the underlying CR mechanism has minimal overlap with the disease mechanism.

How this study scored 54/100
Published in a peer-reviewed journal. Scored zero for study design, effect reported and randomised / blinded because the record does not state it. We have no data on journal's citation record, times cited, citations per year, impact for its field and year, cited by studies here, still being cited, senior author's track record, institution's track record, multi-centre collaboration and free to read, so those were left out: it earned 30 of the 56 points we could judge it on, which is 54 out of 100.
