Alpha-synuclein co-pathology in a real-world early Alzheimer's disease cohort
Tamara Shiner, Talya Nathan, Mori Hai Levy, Aya Bar David, Nurit Omer, Anan Abu Awad, Elissa Ash, Mali Gana Weisz et al.. Alzheimer's & dementia : the journal of the Alzheimer's Association, 2026-03
Topic: Plasma and spinal-fluid biomarkers
Of 65 participants with biomarker-confirmed AD, 21 (32.3%) were αS-SAA positive. Positivity was linked to older age at testing and self-reported olfactory impairment (P = 0.004), but not other demographic or cognitive features. Within the αS-SAA-positive group, RBD presence correlated with faster seeding kinetics. αS-SAA positivity is common in early AD and associated with olfactory dysfunction. Longitudinal follow-up is required to test if assay status predicts response to ATTs.
Key findings
- Of 65 participants with biomarker-confirmed AD, 21 (32.3%) were αS-SAA positive.
- Positivity was linked to older age at testing and self-reported olfactory impairment (P = 0.004), but not other demographic or cognitive features.
- Within the αS-SAA-positive group, RBD presence correlated with faster seeding kinetics. αS-SAA positivity is common in early AD and associated with olfactory dysfunction.
- Longitudinal follow-up is required to test if assay status predicts response to ATTs.

How this study scored 77/100
Published in a leading specialty journal, and it is a longitudinal study following the same people over time. Scored zero for randomised / blinded because the record does not state it. We have no data on journal's citation record, times cited, citations per year, impact for its field and year, cited by studies here, still being cited, senior author's track record, institution's track record, multi-centre collaboration and free to read, so those were left out: it earned 43 of the 56 points we could judge it on, which is 77 out of 100.
