Topic: Amyloid, tau and blood-test diagnosis

Alzheimer disease (AD) is the neurodegenerative condition that occurs in the elderly population above 65 years old and is the major cause of dementia. AD is characterized by memory loss, behavioural changes, and cognitive dysfunction. AD has complex pathophysiology with different mechanisms that include the accumulation of amyloid beta plaques, the formation of neurofibrillary tangles due to the hyper phosphorylation of tau, oxidative stress, and neuroinflammation, dysfunction of the mitochondria, cholinergic insufficiency, and excitotoxcity. Various risk factors include the Apolipoprotein E (APOE) ε4, unhealthy life styles, and environments. The FDA-available drugs include the ones that inhibit the cholinesterase enzyme (donepezil, rivastigmine, and galantamine) or the ones that inhibit the NMDA receptor (memantine), which can decrease the effect but not prevent the disease. Emerging approaches include anti-amyloid antibodies, BACE1, gamma-secretases, anti-tau therapies, stem cell, and gene therapies. Plant medicines have been used for centuries; currently they have been used to cure or prevent AD. The plant has neuroprotective, antioxidant, anti-inflammatory, anti-amyloid, and anti-tau effects. Some phytoconstituents have shown the positive effect on the disease-related pathways: Curcumin, resveratrol, backsides, and withanolides. The current review shows the multifactorial aspect of Alzheimer disease, the summary of the current and new therapeutic methods, and the potential of medicinal plants and their phytoconstituents as safer and efficient alternatives to the current treatment of Alzheimer disease

Key findings

Why it matters

This review states that currently FDA-available drugs for Alzheimer's disease can decrease symptoms but not prevent the disease. It lists emerging approaches such as anti-amyloid antibodies, anti-tau therapies, stem cell and gene therapies, and plant compounds, but presents no confirmed cure or prevention.

Published in a peer-reviewed journal. Scored zero for study design, sample size, effect reported, studied in people and randomised / blinded because the record does not state it. We have no data on senior author's track record, so that was left out: it earned 26 of the 96 points we could judge it on, which is 27 out of 100.
