Beyond “Denial”: Understanding and Responding to Help-Refusal in Dementia
Carey P.. Preprints.org, 2026-10-26
Topic: Health services, long-term care and technology
Evidence for interventions that reduce refusal is strongest for music during personal care and modified bathing methods, with some support for positive communication and person-centred approaches, though quality is generally low and few studies involve family caregivers in community settings. Help-refusal should be understood as a symptom and a relational challenge rather than a character flaw, and as something to understand rather than merely to manage. This article proposes a staged, non-coercive framework (Understand, Check, Frame, Partner, Plan, Escalate, Sustain) that keeps professional assessment central, protects dignity and autonomy, and supports caregiver well-being. The framework is a conceptual synthesis and has not been empirically tested. A plain-language guide for families is provided in Appendix A.

Key findings
- Evidence for interventions that reduce refusal is strongest for music during personal care and modified bathing methods, with some support for positive communication and person-centred approaches, though quality is generally low and few studies involve family caregivers in community settings.
- Help-refusal should be understood as a symptom and a relational challenge rather than a character flaw, and as something to understand rather than merely to manage.
- This article proposes a staged, non-coercive framework (Understand, Check, Frame, Partner, Plan, Escalate, Sustain) that keeps professional assessment central, protects dignity and autonomy, and supports caregiver well-being.
- The framework is a conceptual synthesis and has not been empirically tested.
- A plain-language guide for families is provided in Appendix A.

How this study scored 16/100
Preprint — not yet peer-reviewed. It scored 21 on the same rubric, but non-peer-reviewed sources are capped at 16, so it shows as 16. Scored zero for study design, sample size, effect reported, studied in people and randomised / blinded because the record does not state it. We have no data on times cited, citations per year, impact for its field and year, cited by studies here, still being cited, senior author's track record, institution's track record and multi-centre collaboration, so those were left out: it earned 13 of the 62 points we could judge it on, which is 21 out of 100.
| Component | Points | Why |
