Molecular imaging with Pittsburgh Compound B confirmed at autopsy: a case report
Bacskai BJ, Frosch MP, Freeman SH, Raymond SB, Augustinack JC, Johnson KA, Irizarry MC, Klunk WE et al.. Archives of neurology, 2007-03-01
Topic: Brain proteins, microglia and lab models
Objective To determine the correspondence between uptake of Pittsburgh Compound B (PiB) in life and measures of beta-amyloid (Abeta) in postmortem tissue analysis. Patient A 76-year-old man with a clinical diagnosis of dementia with Lewy bodies underwent fluorodeoxyglucose (18)F and PiB positron emission tomographic brain scans. Imaging revealed marked region specific binding of PiB and abnormal fluorodeoxyglucose uptake.

Key findings
- Patient A 76-year-old man with a clinical diagnosis of dementia with Lewy bodies underwent fluorodeoxyglucose (18)F and PiB positron emission tomographic brain scans.
- Imaging revealed marked region specific binding of PiB and abnormal fluorodeoxyglucose uptake.
- Intervention Autopsy was performed 3 months after the PiB scan.
- Results Autopsy confirmed the clinical diagnosis; in addition, there was severe cerebral amyloid angiopathy and only moderate numbers of parenchymal Abeta plaques.
Why it matters
In one 76-year-old man diagnosed with dementia with Lewy bodies, a Pittsburgh Compound B (PiB) amyloid scan showed "marked region specific binding", and autopsy 3 months later confirmed the Lewy body diagnosis. The autopsy also found "severe cerebral amyloid angiopathy" (amyloid in blood-vessel walls) but "only moderate numbers" of amyloid plaques. This is a single case: it cannot show how accurate PiB scans are in general, and it tests no treatment.

How this study scored 51/100
Published in a peer-reviewed journal. Scored zero for study design, sample size, effect reported and randomised / blinded because the record does not state it. We have no data on journal's citation record, so that was left out: it earned 49 of the 96 points we could judge it on, which is 51 out of 100.
