CMS GUIDE Model participant footprint by state (August 2026 participant list)

Measured in-effect Care model
  • Enacted 2024
  • Effective 2024

CMS's GUIDE participant list (xlsx, 'Last updated August 2026') contains 474 GUIDE IDs; each lists the state(s) in which it provides services (participants may serve multiple states or serve virtually). Count of participants listing each state: AK 27, AL 27, AR 39, AZ 45, CA 70, CO 36, CT 50, DC 19, DE 35, FL 88, GA 58, HI 28, IA 55, ID 33, IL 50, IN 49, KS 26, KY 55, LA 26, MA 38, MD 46, ME 16, MI 45, MN 37, MO 41, MS 37, MT 23, NC 62, ND 18, NE 26, NH 25, NJ 65, NM 37, NV 34, NY 83, OH 64, OK 30, OR 44, PA 121, RI 20, SC 54, SD 24, TN 47, TX 69, UT 42, VA 66, VT 27, WA 39, WI 49, WV 29, WY 36; territories AS 8, GU 1, MP 1, FM 1, MH 2; 'Not available' 20.

Who it covers: Medicare beneficiaries with dementia in the states served by 474 listed GUIDE participants

(count of rows in CMS's own list, computed 2026-09-22 by splitting the 'State(s) in which Participant Provides Services' column on ';'). Counts sum to more than 474 because many participants list several states. NOTE: the GUIDE web page shows 'Number of Participants: 292' while the August 2026 xlsx has 474 GUIDE IDs; the discrepancy is CMS's and is left unresolved here.

CMS final decision CAG-00431R: removal of NCD 220.6.20 (Beta Amyloid PET in Dementia and Neurodegenerative Disease)

Measured repealed Coverage
  • Enacted 2023
  • Effective 2023

Final decision dated October 13, 2023 removed the 2013 NCD that had limited amyloid PET to one scan per patient under coverage with evidence development. Coverage decisions for amyloid PET are now made by Medicare Administrative Contractors under Section 1862(a)(1)(A), which may cover more than one scan per patient (e.g., for anti-amyloid treatment eligibility and monitoring). Proposed decision was posted July 17, 2023.

Who it covers: Medicare beneficiaries being evaluated for Alzheimer's disease (amyloid PET)

/COVERAGE RULE. Status 'repealed' refers to the NCD (the original 2013 CED NCD, effective 2013-09-27, was removed); the effect is broader coverage, not less. CMS declined to make the decision retroactive.

CMS National Coverage Determination 200.3: Monoclonal Antibodies Directed Against Amyloid for the Treatment of Alzheimer's Disease (coverage with evidence development)

Measured in-effect Coverage
  • Enacted 2022
  • Effective 2022

Effective April 7, 2022, Medicare covers FDA-approved anti-amyloid monoclonal antibodies under coverage with evidence development (CED). Antibodies approved on a surrogate endpoint (amyloid reduction) are covered only in randomized controlled trials; antibodies approved on a direct measure of clinical benefit (e.g., lecanemab, donanemab) are covered in CMS-approved prospective comparative studies including registries, and in NIH-supported trials. Amyloid PET performed under an approved study protocol also satisfied the amyloid PET CED requirement.

Who it covers: Medicare beneficiaries with a clinical diagnosis of MCI due to AD or mild AD dementia with confirmed amyloid pathology

/COVERAGE RULE. Registry-based coverage for traditionally approved drugs has been operating since the July 2023 lecanemab traditional approval (see our cost data coverage rows for VA and CMS context). NCD decision memo is CAG-00460N (ncaid=305).

Measured enacted Caregiver support
  • Enacted 2020
  • Effective 2020

Section 202 amends the Older Americans Act (sections 302(3) and 711(6)) by inserting 'of any age' so that individuals with Alzheimer's disease and related disorders who are younger than 60 become eligible for OAA supportive services and family caregiver support programs. The stand-alone Younger Onset Alzheimer's Disease Act (S.901, Sen. Collins, introduced 2019-03-27) was folded into this reauthorization.

Who it covers: US individuals with Alzheimer's disease of any age (extends Older Americans Act supportive services and National Family Caregiver Support Program eligibility below age 60)

H.R.4334 sponsor Rep. Suzanne Bonamici; introduced 2019-09-16; latest action 2020-03-25 'Became Public Law No: 116-131'. Sec. 202 text read directly from the enrolled law. The same law authorizes OAA Title III appropriations FY2020-FY2024 (e.g. Supportive services $412,029,180 for FY2020) but those are not dementia-specific so the dollar value is left empty.

What could make these numbers misleading

Dollar figures are the amounts stated in the source (an appropriation, a budget line, or a bill text), for the fiscal year shown. An 'in-effect' status reflects the source at the time it was read, not a live legal check.

Every figure on this page is read straight out of a file in our data folder; nothing is typed in by hand. You can download the raw data and check any row yourself. This is a summary of published research, not medical advice.
